To initiate service, please complete the referral form. A representative will call within one business day to confirm the request. If you can come to any of our offices, an appointment will be scheduled at that time. If you need onsite/office or home/community-based service and are enrolled in the Medicaid program, an appointment will be scheduled within two business days.
Your answers to the referral form’s questions are kept in the strictest confidence and secured and will be used to assign a clinician with experience in working with your specific needs.
