Health homes are holistic services that cater to the holistic needs of an individual. Individuals eligible for this service are those who have two or more conditions, which the criteria also include mental health conditions. Our professionals are knowledgeable about effective care coordination and health promotion.
We also know that in dealing with a behavioral health condition, having a strong support system is important. That is why in our services, we also offer client and family support. Family support allows family members to be educated on the nature of their loved ones’ condition and the ways to support them in their commitment to their treatments. We can also provide referrals to their communities & social support services so they can have access to important resources.
